Wide-Awake Hand Surgery with Andrew Yang, MD

Dr. Andrew Yang is a hand surgeon with fellowship training in hand and upper extremity surgery at Rothman/Jefferson and board certification in orthopaedic surgery by ABOS. For selected hand procedures, wide-awake local anesthesia may be an option after an individual evaluation.

Overview

Wide-awake local anesthesia, often called WALANT, uses local anesthetic to numb the treatment area while the patient remains awake. The WALANT approach does not use general anesthesia or sedation. The anesthesia plan is confirmed for each procedure.

When a procedure is being considered

  • WALANT is an anesthesia approach, not a diagnosis. It may be discussed when a hand procedure is being considered.
  • The condition's symptoms and treatment choices are evaluated before deciding on an anesthesia plan.

Evaluation and planning

The clinician reviews the diagnosis, proposed procedure, health history, medications, and patient preferences. The surgical team confirms whether WALANT and its preparation instructions are appropriate.

Non-surgical options

  • Many hand conditions have non-surgical options that may be considered first.
  • The need for surgery and the anesthesia approach are separate decisions made after evaluation.
  • WALANT is not appropriate for every patient or procedure.

How wide-awake local anesthesia works

  • WALANT may be used for selected carpal tunnel, trigger finger, De Quervain's, tendon, and other hand procedures.
  • Local anesthetic is used without general anesthesia or sedation.
  • In many cases, pre-operative fasting or a ride may not be needed; follow the instructions from the surgical team for your specific procedure.

Who may be a candidate

A candidate is identified through an individual review of the condition, planned procedure, health history, medications, and preferences. The surgical team explains the anesthesia plan and any preparation needed.

Recovery timeline

  • The team reviews the procedure and instructions on the day of surgery.
  • After the procedure, follow dressing, motion, and activity guidance.
  • Light use and return to daily tasks depend on the procedure.
  • Recovery varies; keep follow-up appointments and contact the office with concerns.
